I'm running qmail-scanner-1.23 and qmail-ldap-1.03-20040401.patch spamassasin 2.64

We have the same problem, but we don't use qmail-ldap but vpopmail.

That means, it's probably not qmail-ldap or vpopmail causing the problem, but the spamassassin or qmail-scanner. I would guess that it's spamassassin, but that's only a guess. I don't have the time to investigate that further, I simply delete the mails I get like that - but that should be an hint where to look for the problem.
