We have the same problem, but we don't use qmail-ldap but vpopmail.

Here one example-mail (it's complete like I posted it here, no body):

That means, it's probably not qmail-ldap or vpopmail causing the 
problem, but the spamassassin or qmail-scanner. I would guess that it's 
spamassassin, but that's only a guess.
I don't have the time to investigate that further, I simply delete the 
mails I get like that - but that should be an hint where to look for the 
problem.
