[RCSE] Updated Survey Form

Just wanted to let everyone know that as suggested I have updated the
Sailplane Survey to include wingeron and flaperon control selections.

Please submit your reviews of your planes so that we can amass a great
database.

It only takes 30 seconds or so for each model!!

Thanks to all who have submitted planes already - I have about 45 entries so
far in the Survey. I should be able to get a page setup this weekend so
everyone can review the survey results to date.
